"The Affidavit of Vaccine Administration Safety, Efficiency, and Liability" sample form is an affidavit requesting the vaccine administrator to declare under penalty of perjury that the vaccine(s) he or she will be administrating are 100% safe and that the administrator will be held personally liable for any adverse side effects or harm to your child(ren) resulting from the vaccines injected into your child.
